Change of Name - This process is to change your legal name

This process is for students that need to change their legal name. This will affect all legal documents such as your tax document and transcripts.

If you are looking to change your chosen name click here - this does not change your name on legal documents

You will need to upload one of following:  

  1. Valid driver’s license 
  2. Wedding license
  3. Court documentation

Students that need to submit forms, can login to the Bruin Portal and click on eForms

RG - Change of Name

This is to change your legal name in the system, this new name will be used for official documents such as Student Transcripts, Tax documents and so on.

Please use the ‘Attachments’ function at the bottom of this form to provide one of the items listed below as proof of your name change along with a picture ID for verification purposes. 

Once etrieve Central is open:

  1. Click on Forms
  2. Click on the RG - Change of Name
  3. Fill out the form on the right, enter your New Name 
  4. You will need to submit an attachment.

To add an Attachment 

  1. Click on Upload Files
  2. Navigate to your file to upload and select it
  3. Click on Open
  4. Click on Submit once finished with the Form and Attachment
